外來人口換發「新式統一證號」資料變更提醒事項

1. 內政部推動外來人口換發「新式統一證號」政策，自110年1月2日起開始進行外來人口新式統一證號換發作業：
The Ministry of the Interior has promoted the policy of renewal of "New UI No." for Foreign Nationals and carried out this task from January 2, 2021
(1)請至內政部移民署全球資訊網「新式外來人口統一證號專區」查詢相關資訊(如：政策說明、中英文版常見問答及多國語音懶人包等；網頁連結：https://www.immigration.gov.tw/5385/7445/238440/ )。Please refer to the "New UI No. Format for Foreign Nationals " at the Website of National Immigration Agency, Ministry of the Interior to search for relevant information ( e.g., Policy Explanation, FAQ in Chinese and English version and a short version for Multilanguage; website link:“https://www.immigration.gov.tw/5385/7445/238440/”).
(2)亦可請您撥打「內政服務熱線(電話:1996)」或「外來人士在臺生活諮詢服務熱線(電話:1990)」洽詢。
You may also call "MOI Service Hotline (1996) " or " Foreigners in Taiwan Hotline (1990)" for consultation service.
2.若您已換發新式統一證號者，提醒您申請保單的ID變更:
If you have already been issued a new UI Number, you are reminded to apply the ID change of your policy:
(1)連繫保單服務人員服務送件。
Contact your service agent in order to submitting the application.
(2)請列印「契約內容變更/復效/復繳暨保險單補發申請書(繁式)」(https://www.fubon.com/life/cms/26316A2EBF774B809CD0859A39B8D5E2/2025-01/202501031536373964172542.pdf) 或洽本公司客服中心：0809-000-550索取變更申請書，填妥資料並親自簽名，並檢附「最新身分證件(居留證)」正、反面影本，郵寄至本公司(104 台北市中山區建國北路二段236號)辦理統一證號變更作業事宜。Download and print the “Application Form for Policy Change＂(https://www.fubon.com/life/cms/26316A2EBF774B809CD0859A39B8D5E2/2025-01/202501031536373964172542.pdf)) or call our customer service hotline at 0809-000-550 to request the ” Application Form for Policy Change”. To change your UI Number, please fill in the Application form and sign in person, and mail it to our company(No. 236, Sec. 2, Jianguo N. Rd. Zhongshan Dist., Taipei City 104 , Taiwan (R.O.C.)) with a photocopy of your new UI No. identity certificate (Residence Permit).
(3)請本人攜帶換發之「最新身分證件(居留證)」正本，親臨本公司任一服務櫃檯辦理統一證號變更作業事宜。
Bring the original copy of your “new UI No. identity certificate (Residence Permit)” to one of our Customer service counter to change your UI Number.
